TARRANT COUNTY IMMUNIZATION CLINIC IS COMING TO CMS!

WHO:                  TARRANT COUNTY IMMUNIZATION CLINIC

WHAT:                Tetanus Booster Vaccine  (Tdap)
                             Meningococcal Vaccine (Meningitis)
                             Human Papillomavirus Vaccine (HPV)
                             Hepatitis A Vaccine
                             Hepatitis B Vaccine

WHEN:                THURSDAY, MAY 29 AT 9:00 in the morning

WHERE:             CARROLL MIDDLE SCHOOL CAFETERIA

HOW:                  The permission and vaccine information forms are available from CMS Nurse, Betty Sullivan.  You must sign a separate permission form for each vaccine to be given. Return signed permissions and payment to Nurse Sullivan by TUESDAY, MAY 27.

HOW MUCH:      $14 FOR ONE (1) VACCINE
                             $20 FOR TWO (2) OR MORE VACCINES
                             PLEASE MAKE CHECKS PAYABLE TO TCHD

There is no income requirement—every student is eligible!  If your student will be due for his/her 10 year Tetanus booster within the next year this clinic is a very convenient way to complete this Texas requirement for school attendance.

Choir News!!! The 8th grade-Varsity Treble choir received 4-superior ratings, and 2-excellent ratings at UIL March 11 at Liberty High school in Frisco, earning CMS a UIL Superior plaque in Sight-reading.This group also received a UIL award for CMS last year as 7th graders in 2007. The 7th grade Non-Varsity also choir earned 1 superior, and 2 excellent ratings in concert. Great start 7th graders!
