In accordance with the Texas Administrative Code, section 97.66, no
student will be admitted to CISD without written evidence of immunizations
that are in compliance with the Texas Department of Health regulations.
Provisional Enrollment:
A provisional 30 day enrollment allows a student to enroll in CISD
under the following situations:
• Students in progress:
Students who have received at least one dose of each specified age
appropriate vaccine required by the TDH rules. To remain enrolled,
students must complete the required subsequent doses of each vaccine
series on schedule as rapidly as medically feasible and provide acceptable
evidence of vaccination to the school nurse. If, at the end of the
30 day period, a student has not received a subsequent dose of vaccine,
the student is not in compliance and the school shall exclude the
student from school attendance until the required dose id administered.
• Transfer students:
When a student transfers from a Texas school to CISD, a grace period
of no more than 30 days may be allowed at the new school while awaiting
the transfer of the immunization record, during which time the student
may be provisionally enrolled. It is the parent’s responsibility
to ensure that the records are received.
Note: There is no grace period for students enrolling
from out of state. It is the parent’s responsibility to provide
documentation of required immunizations upon enrollment.
Exemptions:
Chapter 97.62, of the Texas Administrative Code, describes the conditions
under which individuals can seek exemptions from Texas immunizations
requirements. Exclusions from compliance are allowable on an individual
basis for medical contraindications and reasons of conscience, including
religious belief.
•
To claim an exclusion for medical reasons, the parent must submit a
statement, signed by the student’s physician, which states that,
in the physician’s opinion, the vaccine required id medically
contraindicated or poses a significant risk to the health of the student.
Unless, it is written in the statement that a lifelong condition exists,
the exemption statement is valid for only one year from the date signed
by the physician.
•
To claim an exclusion for reasons of conscience, including religious
belief, a signed affidavit must be presented by the student’s
parent/guardian, stating that the student’s parent/guardian declines
vaccinations for reasons of conscience, including because of the person’s
religious beliefs. The affidavit will be valid for the time designated
on the bottom of the Texas Immunization Exemption Form. The student,
who has not received the required immunizations for reasons of conscience,
including religious beliefs, may be excluded from school in times of
emergency or epidemic declared by the commissioner of public health.
? To claim an exclusion for the above, one may only obtain an affidavit
form by submitting a written request with the student’s full
name and date of birth to:

Notification
Routine:
Parents/guardians will be notified via the School Nurse through CISD publications,,
Campus publications, and the CISD web site of Immunization Requirements.
Prior to Exclusion:
The parent/guardian will be notified by the School Nurse through personal contact
and/or a simultaneous written notice (via US Mail) prior to the expiration
of the 30 day grace period.
The parent/guardian will be notified by the School Nurse through personal contact
and/or simultaneous written notice (via US Mail) prior to non-compliance that
a required immunization has not been completed on the medically recommended
schedule.
Exclusion:
A student will be withdrawn from school by the Campus Principal when:
• No immunization documentation has been received from the previous Texas
school within the 30 day provisional enrollment. The parent/guardian will have
one
week after the final notification to provide documentation.
• The required immunization series is not completed on the medically recommended
schedule. The parent/guardian will have one week after final notification to
provide documentation.
Students presently enrolled in CISD
August enrollment for non-compliant students will be delayed until
the school receives written evidence of immunization compliance.
When non-compliance occurs during the school year the above notification
and withdrawal procedures will be followed.
