About The Nurse


Mrs. Betty Sullivan has been a School Nurse with CISD since 1991.  Her clinic is located adjacent to the main office.  She can be reached during school hours via email at: sullivb@cisdmail.com or direct telephone line: 817-949-5402.

Goals of the School Health Services Program

School health services are offered to help CMS students and staff maintain an optimum level of wellness.  Identification of health related barriers to learning are crucial to the provision of an appropriate and excellent educational experience.

FAQ

What if my student must take medications at school?


For medications given at school for more than 10 consecutive days a written physician's statement must be submitted to the health clinic which provides the name of the student; name of the drug; the dose; the times the drug is to be taken; and the diagnosis or reason the medication is needed; and the duration of the physician's order.

All prescription medication must be in the original container labeled by the pharmacy.  All over-the-counter medication must be in the original container. All over-the-counter medication must be supplied by the parent.

The parent or guardian must complete a CISD medication permit obtained from the campus nurse for all medications given at school.

At the end of the school year any medication remaining in the clinic will be destroyed.

What if my student is ill and unable to attend school?

Please notify the school first thing in the morning (817-949-5405) if your student is going to be absent for any reason.

What if my student becomes ill at school?

Students who become ill at school must be seen by the school nurse. The nurse will contact the parent if the student is too ill to stay in school.

Students who vomit or have a temperature of 100* are sent home.

A parent or authorized person must sign-out the student through the Attendance Office.

What if my student has an allergy?

Please notify the nurse of any allergies that may require medication or emergency treatment during the school day. If your student needs medication to treat an allergy OR if your student is NOT to eat certain foods at school notify the Mrs. Sullivan AS SOON AS POSSIBLE.

Your student may carry an Epi-Pen with doctor's permission on file in the health clinic.

What if my student has asthma?

Please notify Mrs. Sullivan if your student has asthma.  Your student may carry an inhaler with doctor's permission on file in the
health clinic.

What if my student has diabetes?

Please notify Mrs. Sullivan if your student has diabetes. Arrangements can be made to familiarize school staff with your student's diabetic protocol.

What if my student is an epileptic?

Let Mrs. Sullivan know that your student is an epileptic. She will make arrangements to familiarize school staff with your student's epilepsy protocol.

Will my student's health information be shared with the school staff?

Mrs. Sullivan will only share health information with the staff designated by you.  School staff is aware of the sensitive nature of some health information and keeps all student health information confidential.
