All immunizations must be completed by the first date of attendance.

The law requires that students be fully vaccinated against the specified diseases. A student may be enrolled provisionally if the student has an immunization record that indicates the student has received at least one dose of each specified age-appropriate vaccine required by this rule. Instructions for requesting the official exemption affidavit that must be signed by parents/guardians choosing the exemption for reasons of conscience, including a religious belief, can be found at www.dshs.texas.gov/immunize/school/exemptions.aspx . The original Exemption Affidavit must be completed and submitted to the school. Texas DSHS staff continue to process conscientious exemption requests. Exemption requests are processed in the order they are received and it may take up to 3 weeks for requested affidavits to be mailed.

Classifications of Residency and Required Documentation:

1.  For an established resident residing in a home owned within Carroll ISD boundaries:

You may register online for the campus your home is zoned for with the following proof of residency:

• A copy of your most recent property tax statement indicating home ownership AND
• A current utility bill (electric or water) indicating the home address and the parent's name
• A copy of enrolling parent/guardian's driver's license

2.  For residents that have recently completed the purchase of an existing home in Carroll ISD boundaries:
You may register online for the campus your home is zoned for with the following proof of residency:

• A copy of the complete (all pages) final/signed Closing Disclosure, Warranty Deed, or HUD Settlement Statement received from your Title Company, including signature pages AND

• A copy of any water or electric deposit receipt; or a copy of your most current electric or water bills

• A copy of enrolling parent/guardian's driver's license

3. For new residents leasing a home in Carroll ISD boundaries:
You may register online for the campus your home is zoned for with the following proof of residency:

• A copy of a valid, fully executed lease agreement (must be complete agreement with signatures from both tenant and landlord) AND

• A copy of the current electric and water bills or any electric or water deposit receipt AND

• A copy of enrolling parent/guardian's driver's license

4. For new/renewing residents living with another family and do not rent/own in Carroll ISD boundaries:
You may register online for the campus your home is zoned for with the following proof of residency:

• Affidavit of Residency notarized form 

• A copy of a valid, fully executed lease agreement with all occupants listed (must be complete agreement with signatures from both tenant and landlord) AND

• A copy of the current electric and water bills or any electric or water deposit receipt AND

• A copy of enrolling parent/guardian's driver's license

City of Southlake/Town of Westlake Open Enrollment: Open enrollment is available for Southlake/Westlake residents and Southlake First Responders (Police, Fire, EMT) only. Transportation is not provided to students enrolled on a transfer.

• Students- Grandparent In-Resident *Additional Student Transfer Application must be submitted on the Enrollment Dashboard (CISD website)

Additional Residency Details: In order to enroll in the pre-kindergarten program, families are required to submit documentation confirming their current residency within Southlake or in a residence zoned to CISD. For those in the process of purchasing, constructing, or leasing a property, enrollment in the pre-kindergarten program will be considered upon relocation to the designated residence, supported by utility bills and either a deed or lease agreement.

Carroll ISD offers High-Quality Prekindergarten with the addition to three High-Quality Prekindergarten co-teach classrooms. Prekindergarten co-teach  serves three types of early education learners in an inclusive environment with a general education prek teacher,  prek special education teacher and a paraprofessional. The benefit of a co-teach classroom is the increased opportunities for individualized instruction. With two teachers in the classroom, students can receive more individualized attention and support. 

• Students who are eligible for Early Childhood Special Education (ECSE) formerly PPCD
• Students who are meet state Eligible requirements for free public school Prekindergarten 
• Tuition-Based Students (based on space availability) 

The Prekindergarten program is currently provided at three elementary campuses: Carroll Elementary, Rockenbaugh Elementary, and Old Union Elementary.

DRAGON U

To change phone numbers or addresses in Skyward, you can fill out the form below and submit it to your the campus(es) or Student Services.

Change of Address/Telephone

• A parent/guardian requests an account through Skyward: New Student Online Enrollment located on the Enrollment Dashboard.
• A verification email is sent to the parent/guardian with login and password information. Please check your spam/junk folder if email is not received.
• The parent/guardian logs into CISD’s Enrollment Access within Skyward Family Access with the login and password provided in the verification email
• The parent/guardian completes the student application process, uploads requested documents, and completes required district forms (documents requested include proof of residency (student's birth certificate, social security card, immunizations, last report card or transcript for students enrolling in Grade 1 and above, withdrawal form and enrolling parent's drivers license).

• The appropriate campus reviews the application and communicates the next step to the parent/guardian

• Once the application is approved, the student is enrolled into CISD at the appropriate campus

Pending Construction/Lease Agreement:

• A nonresident student whose family is building, purchasing, or leasing a house within District boundaries may be permitted to attend District schools prior to establishing residency. The parent/guardian will need to complete an Online Transfer Application for each student. Documentation confirming that the student will be establishing residency may be authenticated using one of more of the following:

  • A fully executed lease or rental agreement indicating the date of anticipated occupancy of the dwelling.

  • A valid, fully executed, real estate earnest money contract or a contract of sale with specified date on which construction began and the date of closing

  • Any other documentation acceptable to the Superintendent that establishes bona fide proof of intended residence of the student.

If a parent/guardian does not own or rent/lease their current residence, cannot provide proof of residency at their current address in their own name, and they are currently living in the home of someone that is NOT the parent/guardian of the student that is not due to a homeless or temporary living situation, the parent/guardian will be required to provide a notarized Affidavit of Residency Form (must be completed with a CISD Notary)in addition to the required residency documents. • This will require the resident to confirm the status of the applicant residing with them and they will need to submit all needed paperwork confirming proof of residence in the Carroll Independent School District.

If only the student is residing at the residence then we also require:

• Limited Power of Attorney (Power of Attorney Documentation must be signed and notarized by a CISD notary with both the CISD resident and parent/guardian)

A nonresident student who is the grandchild of a District resident shall be eligible to attend District Schools on a tuition-free basis. Verification of the grandparent/grandchild relationship between the District resident and the student shall be in accordance with administrative regulations. The parent/guardian will need to complete the Online Transfer Application  annually for Grandparent Affidavit.

Information for Families Enrolling from Homeschool/Non-Accredited Schools

Placement for Grades K-8

A student enrolling in CISD from a non-accredited public, private, or parochial school, including homeschool, shall be placed initially at the discretion of the principal, pending observation by classroom teachers, counselors, and the principal. Factors considered in placement may include:

• Any academic records or grades provided by the sending school
• Appropriate age/grade placement 
• Social-emotional development of the student 
• Previous grade levels and enrollments 
• Parents may also provide any syllabi or curriculum links to the homeschool or non-accredited entity
• Required placement testing 

Enrollment of Students in 9th Grade

If a student does not have a record showing promotion from 8th to 9th grade from an accredited school, the student will enroll at the middle school they are zoned to attend.  Once the student is enrolled, they will take Credit By Exams for the following core subjects: 8th English Language Arts, 8th Math, 8th Science, and 8th Social Studies.  To be promoted to 9th-grade students must earn a 70% or above on all exams.

If a student does not meet these testing standards, the campus will look at the placement considerations outlined above in grades K-8, and the final placement decision will be made at the discretion of the campus administration.

Grade 9-12 High School Credits

When a student enrolls, any credits awarded by a non-accredited entity, including homeschool, will need to be verified by utilizing credit by exam (CBE).  All students will be provisionally enrolled based on previous academic records and will be CBE tested using TEKS-aligned, local assessments to verify credits.  Students will not be allowed to enroll in classes with a prerequisite in which the prerequisite has not been verified by a CBE.

Recommendations from TEA regarding Homeschool: TEA and Homeschool

Yes, Carroll ISD offers a yearlong foreign exchange program.  We do not admit students for one semester only. We only accept 11^th graders into our program, and students must return to their home country to graduate from high school.

All foreign exchange students must go through an accredited foreign exchange agency to be considered.  The agency will assist them with the Visa process. Our program accepts a maximum of five students per year to participate.

We would suggest contacting an accredited foreign exchange agency, as most local programs require an agency for placement. These following agencies have represented some of our current exchange students, and will be able to guide them through the process.

If a parent/guardian with joint or sole custody of a student lives in CISD boundaries but the student resides outside the district, you will fill out the Online Transfer Application and select the Guardian in Residence option for enrollment. For any questions, please contact Student Services.

• Guardian in Residence is defined by TEA under TEC, §25.001(b) as:  Student does not reside in your school district but has a parent who does and that parent is a joint or the sole managing conservator or possessory conservator of the student.

Student Teaching / Classroom Observation

Please review the CISD Board policy, which will allow homeschooled students zoned to CISD to participate in UIL activities. 

• Students must submit written intent to participate in UIL activities no later than 3:00PM CST on the last day of the academic school year prior to the year in which they wish to participate to student.services@southlakecarroll.edu. Late submissions will not be accepted.
•  Student Services will confirm residency information and notify parent/guardians to move forward to the next step of enrollment.
• Once the student is registered, Athletics will be notified to contact the family for the final steps needed to participate in UIL activities.
